Convert M2 format

A quick routine to convert Glenn's M2 data into the format needed for my new form M2, and for Cookie's system.

User operation...

1. In Excel, select your worksheet - CTRL-A, CTRL-C. Important... When combining spreadsheets for the current year and the following year, the most recent records must be at the top of the list. The program discards any duplicates that it later encounters, and so these duplicates need to be earlier transaction records.
2. Paste the data into the textarea box below; CTRL-V
3. Click the Submit button. The results will be displayed in a table at the bottom of the page.
4. Either... Select and paste the displayed results - CTRL-A, CTRL-C. Then go paste the data somewhere.
5. Or... Tick the mail box, enter a valid email address and click the Submit button again. This will email the file. (Debugging: if you do not enter an email address, the email will be displayed on the screen.
